Job summary

The Eligibility position provides Patient Care Support to our Primary Care department handles Eligibility - Registrations - and Enrollments needs and also used to assist multiple clinics in primary care.

This position is vital in supporting the outpatient clinic team to effectively carry out day-to-day operations to include scheduling for all primary care clinic The Advanced MSA provides specialized and expert administrative patient support while working collaboratively in an interdisciplinary coordinated care delivery model Work involves specialized administrative judgment and the flexible use of a wide range of clinical flow processes relating to access to care across multiple clinics specialties and/or care in the community resources Recommends changes to existing clinic procedures based on current administrative guidelines Coordinates with the patient care team to review clinic appointment availability utilization ensures that clinic schedules are closely monitored to effectively support the needs of the clinics makes adjustments as necessary MSAs at this level develop maintain effective efficient communication with patients interdisciplinary coordinated care delivery model teams VA medical centers other agencies prepare correspondence notify patients of normal lab results manage system follow-up such as consults tests etc Other assignments at this level include but are not limited process incoming patient secure messaging through MyHealthyVet coordinates with healthcare providers participates in team huddles meetings manage plan problem solve follow-up with patients shares information collaborating interdisciplinary team sets priorities deadlines adjusts work flow meet team patient needs identifies incomplete encounters communicates findings providers assists reinforce plans self-help solutions enters appropriate information electronic record monitors pre-appointment information requirements assures readiness for visit/procedure manages systems verifies validates accuracy resolves issues evaluates patent information schedule lists determine whether requires immediate appointment informs members about shared patents those receive their
