Founded as a result of the federal Older Americans Act, the AAAs are mandated to "facilitate the area-wide development and implementation of a comprehensive, coordinated system for providing long-term care in home and community-based settings, in a manner responsive to the needs and preferences of older individuals and their family caregivers." PCA carries out this mission through advocacy, stewardship of public funding, planning, development and administration of programs, and service coordination.
The mission of PCA is to improve the quality of life for older Philadelphians and those with disabilities and assist them in achieving the greatest possible levels of health, independence, and productivity.

RequirementsDescription of Role:
Reporting to a Care Manager or a Care Management Supervisor, this position is responsible for the care management of individuals who have declined OPTIONS or Waiver programs and are receiving home delivered meals only.
The care management aide provides services as specified and required by the PA Department of Aging for "Meals Only" consumers including annual reassessments, home visits, and telephone contacts.
Required Education:
Associate degree (A.A.S) in a human services or related field.
Job Responsibilities:
a. Performs all care management functions for "meals only" caseload. b. Conducts NAT-E assessment of nutrition needs and eligibility for home delivered meals annually.c. Conducts semi-annual home visits and quarterly telephone calls to monitor eligibility, changing needs, and satisfaction with meals.d. Creates service orders for meals in Oracle and SAMS.e. Follows up on meal delivery problems as needed.f. Completes all required documentation and data entry in SAMS and Oracle. f. Other duties as assigned.
Additional Requirements:
Must have a valid driver's license, good driving record, and continuous access to a fully insured car.
