The Home Health Patient Care Manager oversees and coordinates clinical services, offering nursing care to patients at home based on physician prescriptions and in compliance with laws, regulations, and agency policies.

Responsibilities include:

• Providing clinical services within the state laws' defined scope of practice
• Coordinating with the healthcare team and following the care plan
• Managing patient referrals and visits, ensuring proper clinician assignments and orders
• Coordinating patient home health benefits, medical necessity, and insurance approvals
• Continuously assessing patient needs and providing individualized and appropriate care in line with physician orders and home health criteria
• Reviewing assessments and care plans daily, providing recommendations to clinicians as needed

Education & Experience

• Current RN licensure in the state of practice
• Current CPR certification
• Current Driver's License, vehicle insurance, and reliable transportation
