    Description
    Overview
    • Provides direct psychosocial services to mentally ill individuals in the community who are experiencing, or are at risk of, an acute psychological crisis or are in need of mental health treatment. Ensures assessment, linkage, referral, and follow-up with ongoing service providers. Provides information and consultation to community agencies and other disciplines. Assists in the administrative and clinical functioning of the program. Works under general direction.
    Compensation:
    • $70, $87,700.00 Annual
    • Provides clinical supervision and direction to social work/behavioral health team members. Manages program functions in the Program Manager's absence.
    • Organizes program components for staff utilization and management including triage, case management, scheduling, referrals, outreach, evaluations, and liaison activities.
    • Participates in quality assurance activities. Ensures compliance with regulatory requirements.
    • Screens referrals.
    • Performs psychosocial evaluation and assessment of mental health service needs and emergency social service needs. Develops short-term service plans. Provides crisis intervention services and supportive counseling.
    • Prepares and maintains case records. Encourages service-resistant clients to accept mental health services.
    • Participates in team meetings and case conferences. Provides linkage, referral, and information to mental health and social services providers.
    • Coordinates care continuity between clients and service agencies. Provides outreach services to individuals in acute psychosocial crisis.
    • Serves as a resource person for social services available to patients. Participates in in-service training and education.
    • Assists in the functioning of the team. Supports community programs, education, and advocacy.
    • Contributes to clinical and administrative policies. Participates in special projects and other assigned duties.
    Qualifications
    • Licenses and Certifications:
      • Valid driver's license required
      • Licensed Mental Health Professional required
    • Education:
      • Master's Degree in Social work or related field required
    • Work Experience:
      • Minimum three years experience as an MSW in a mental health setting required
