    Home Health Aide CAH

  • Functions as a patient advocate by communicating to the responsible nurse observations made while providing patient care; requests from family / patient etc.
  • Shows genuine concern to all patients, physicians, and customers; acknowledges all customers in need of assistance in a timely manner.
  • Refrains from discussing confidential information in inappropriate areas of the hospital and with those not concerned.
  • Follows Corporate Compliance and HIPPA guidelines.
  • Does not participate in gossip.
  • Practices knowledge of proper chain of command.
  • Ensures patient safety by identifying patients at risk for falls and takes appropriate actions.
  • Assist the nurse with implementation of alternative measures as a means for restraint reduction prior to their placement.
  • Use of equipment to ensure patient safety ex. gait belt, bed side commode.
  • Uses patient identifiers for identifying patient and before treatment, prior to performing any function as evidenced by 0 adverse outcomes.
  • Adheres to Infection Control Policies by utilizing Standard Precautions, Transmission Based Precautions, and good regular handwashing technique, appropriate use of PPE, handles and disposes of infectious hazardous waste appropriately, as evidenced by observation of the Immediate Supervisor.
  • Re-positions the patient at the specified frequency, assists with ambulation, and performs range of motion exercises, as indicated by order.
  • Maintains skin integrity by keeping skin dry, clean, and reporting reddened areas to the responsible staff nurse as indicated by 0 incidents of decubitus development.
  • Demonstrates knowledge of safety and emergency procedures, responsibilities in all code situations, and disaster plans for the Nursing Department.
  • Reports defective equipment immediately.
  • Functions as patient advocate.
  • Participates actively and accurately in shift report as indicated by observation of the responsible staff nurse.
  • Uses non-defensive communication/body language.
  • Completes appropriate documentation accurately and communicates any abnormalities to the staff nurse.
  • Accurately records daily patient care activities, and provides documentation to the Responsible Nurse for inclusion in the patient's chart.
  • Is cognizant of the Patient's requirements relative to his or her age-specific needs in the performance of direct patient care activities.
  • Delivers and gives fluids / nourishment as directed.
  • Utilizes good aseptic technique while performing catheter care and maintains the closed drainage system appropriately as indicated by observation of the responsible Staff Nurse and 0 adverse outcomes.
  • Performs the functions associated with providing personal hygiene, comfort measures, positioning, transporting, and ambulation of the patient.
  • Gives oral hygiene following meals and otherwise as indicated by patient needs.
  • Gives care to hair and nails as indicated.
  • Performs the functions associated with performing treatments; collecting specimens; taking vital signs, height and weight measurements; records accurately and provides to the Responsible Nurse for inclusion in the patient chart.
  • Begins work on daily assignments upon arrival at work and maintains presence in work area to avoid delays in workload or patient/visitor services, as indicated by supervisor observation and expression of co-workers.
  • Organized work load to anticipate needs and able to complete workload timely.
  • Prioritize treatments and procedures as evidenced by no delay in patient standard of care.
  • Reports to work when scheduled. Displays cooperative attitude when schedule assignments or workloads are changed to meet the needs of the department.
  • Exhibits flexibility and adaptability to meet schedule and work load demands as evidenced by Master Schedule.
  • No excessive abscesses or tardiness.
  • Attends mandatory meetings, participates in committees, and continues with education.
  • Seeks additional training for professional growth.
  • Assists with health fairs, community outreach program.
  • Assists with orientation of new personnel as evidenced by employee satisfaction and observation of the Unit/Shift Manager.
  • Active role in preceptorship of new employees.
  • High School graduation or equivalent required.
  • Completion of an approved training program with successful achievement of certification – CNA status required.
  • Maintains a current individual educational record.
  • Must have and must maintain CNA certification in the applicable state.
  • Must provide documentation of renewal of certification when received from applicable state.
  • Maintain current CPR certification.
