    Description:

    • Seeking a Bi-lingual Nurse Practitioner for an Adult Day Treatment Center in Brooklyn and Queens, NY.
    • This is a contract position with a schedule of 08:00AM-05:00PM, three days in Brooklyn and two days in Queens.
    • The role involves collaboration with primary care physicians and interdisciplinary teams to support participant care, focusing on functional, clinical, and psychosocial needs.

    Requirements:

    • Licensed Nurse Practitioner in NY.
    • Licensed Registered Nurse in NY.
    • DEA Certification.
    • AANP or ANCC Board Certification.
    • Minimum of two years in clinical nursing practice.
    • Experience working with frail or elderly populations.
    • Bi-lingual in Spanish with effective communication skills.

    Qualifications:

    • Graduated from an accredited nurse practitioner program.
    • Two years of experience in home care, care management, or related field.
    • Proficient in Microsoft Office.
    • High-level analytical and research skills.
    • Ability to manage multiple projects and meet deadlines.
    • Medicaid and Medicare Provider IDs in good standing.
    • Medically cleared for communicable diseases with up-to-date immunizations.
