Job summary
The Home Health Clinical Referral Coordinator is the primary point person for referrals and intake.You'll collect and verify documentation, coordinate admissions/resumptions of care,
ensure compliance, and maintain accurate EMR/EVV records so services start on time and billing is clean.
Responsibilities
- Collect & verify timesheets from Caregivers, DSPs/clinicians; reconcile with EVV/Sandata and care plans;
& resolve discrepancies before approval. - Collect, double-check & verify service hours by client/payer;
& reconcile with authorizations, visit notes, and EVV;
& sign off prior to billing. - Cover shifts when aides call off (coordinate replacements;
