    Health Educator/Integration Coordinator

    Type: Part-Time (20 hours per week)/ Full-Time (40 hours per week)

    Purpose: Provide and conduct educational and integration of programs offered by the Chinese Community Health Resource Center (CCHRC).

    Duties and Responsibilities:

    · Assist in assessing the educational needs of the target population of CCHRC.

    · Develop, evaluate, and modify educational programs and materials.

    · Coordinate, promote, and implement educational programs.

    · Assist in grant writing.

    · Prepare reports required program, system-wide, local, state, and federal partners.

    · Attend and participate in required meetings.

    · Coordinate health management programs for integration with the Chinese Hospital Health System and other partners.

    · Strengthen coordination and support of integration programs

    · Attend, participate in, co-chair and/or chair meetings.

    · Expected to work weekends as required by community events.

    · Comply with CHA Compliance Handbook including Code of Ethics and all statutes, regulations, and guidelines applicable to federal and state programs. Responsibilities include following the guideline and reporting suspected violations of any statute, regulations, agreements, or guidelines applicable to all healthcare programs.

    · Perform other tasks as assigned.

    Qualifications:

    · Knowledge of principles and techniques in association with public health and/or health education program administration; planning, promoting, implementing and conducting health education and research program; group process; selecting and designing health educational materials.

    · Minimum of one-year experience in adult and group teaching preferred.

    · Minimum of one-year experience in grant writing preferred.

    · Excellent communication and interpersonal skills.

    · Ability to chair a multidisciplinary committee.

    · Ability to function within a multidisciplinary environment.

    · Good coordination and organization skills.

    · Creative in program and educational material design.

    · Required language skills: read and speak fluent Chinese (Cantonese and/ or Mandarin)

    · Cultural sensitivity.

    · Knowledge and use of computer, including Internet navigation.

    · Ability to work independently and as a team.

    · Bachelor's Degree or higher in Health Education, Public Health, Social Work, or related Sciences.
