    Description

    Summary of Position:


    Assists licensed staff in the provision of basic nursing and therapy care in Skilled Nursing, using the application of basic skills, restorative care, utilizing established policies and procedures, ensuring safe and effective care.

    Care is provided based on ages of the population served; Adult through geriatric - 65 years or older. Provides care as part of the multidisciplinary care team.

    Demonstrates the skill necessary to meet the needs of all residents served in Skilled Nursing through support of the nursing process; collects resident care data, reports findings, assists team to meet resident care goals.

    The restorative nursing assistant is under the supervision of the Skilled Nursing licensed staff and therapists and reports to the Skilled Nursing Director or designee.

    Qualifications


    Education:

    • Post high school Vocational/specialized training
    • Completion of nursing assistant course
    Experience


    • LTC and SNF experience required

    Knowledge/Skills/Abilities:

    • RNA experience preferred

    Certifications/Licensure:

    • BLS Certification
    • CA Nursing Assistant Certification Required

    Essential Functions


    • Provides consistently exceptional care at all times.
    • Recognizes and accurately reports relevant resident information to licensed staff.
    a. Reports on all significant changes in resident condition or motivational level.

    b. Notifies OT of feeding problems, referrals and reassessment needs.

    c. Notifies PT of any mobility problems, referral or reassessment needs.


    • Utilizes all therapy equipment in gym safely with residents in collaboration with therapists, according to their restorative orders

    Provides personal care assistance to residents:
    a. Personal hygiene - bathing, bathroom/bedpan, HS care

    b. Positioning

    c. Self-care activities - dressing

    d. Ambulation & transfers

    e. Dining.

    f. Transport


    • Under direction of the therapist, performs routine restorative treatments
    a. Range of Motion

    b. Strengthening Exercises

    c. Ensures placement of restorative devices

    d. Ambulation

    e. Dining.

    f. ADL exercises

    g. Whirlpool treatments


    • Ensures that all aspects of resident care are documented. Documentation is complete, accurate and timely. Documents on the Restorative Maintenance Program in Point Click Care according to each residents' program orders.
    • As directed by licensed nursing & therapy staff, provides care in a manner that is safe and sensitive to the rights of each resident.
    • Assists in dining room for all meals when scheduled.
    • Recognizes need for purpose vs. task when caring for residents
    • Follows all appropriate infection prevention measures in place in the SNF at all times.
    • Attends monthly RNA meetings with RN supervisor and therapists.
    • Responds to the needs of the department by performing other duties, as necessary.
