    Description

    Position Title: Psychiatric Rehabilitation Program (PRP) Case Manager

    General Description:

    The PRP Case Manager is responsible for providing case management services to individuals recovering from mental illness. The Case Manager works with individuals, the service team, and other providers to ensure the services and resources are in place to meet the needs of the individual. Schedule: Full time. 40 hours weekly. This person reports to the Psychiatric Rehabilitation Program Director. FLSA: Non-Exempt

    Required Education, Knowledge and Skill Abilities to Perform Essential Job Functions:

    A Bachelor's Degree plus one (1) year experience in mental health/psychiatric rehabilitation is required or an AA degree with at least two (2) years experience in mental health/psychiatric rehabilitation may be considered. Must demonstrate the ability to work as part of a team and independently. Effective verbal and written communication skills are required. The ability to respect individual choice and the recovery process is essential. A good driving record is required.

    Summary of Essential Functions, Responsibilities and Duties

    • Coordinate program services (Residential, Psychiatric Rehabilitation Program, Health Home, Supported Employment, community housing, external providers)
    • Assist individuals with developing and carrying out long- and short-term goals as well as tracking progress.
    • Engage in day program activities such as group facilitation.
    • Schedule and accompany individuals to medical and behavioral health appointments as needed.
    • Complete contact notes in EHR, Individual Service Plans, Monthly Progress Notes, annual documents, and other documents as needed and/or assigned.
    • Communicate effectively with the Intake Coordinator, Residential Supervisor and Program Director to ensure linkage of resources and services.
    • Ensure initial and ongoing eligibility for entitlements and annual redeterminations.
    • Provide off-site services in setting of the individual's choice (residence, community setting).
    • Provide services in multiple environments (program, home, community).
    • Responsibility for specialized services and needs such as MTA, ID cards and Community Forensic Aftercare Program monitoring.
    • Engage in crisis management and participate in emergency on-call rotation.
    • Utilize computer systems for external/internal communication.
    • Work in conjunction with PRP team and participate in Utilization Review Committee and other meetings as needed.
    • Perform other duties as requested by program supervisors.
