    JOB TITLE: Clinical Care Manager Assistant - LPN

    DEPARTMENT: Clinical Services

    REPORTS TO: Clinical Care Manager - IPUFriday-Sunday 12 hour day shift

    POSITION SUMMARY:

    Reporting to the Clinical Care Manager, the Clinical Care Manager Assistant (CCMA) is responsible for assisting the manager and the clinical staff in ensuring the smooth functioning of the teams. The CCMA facilitates the timely completion of assigned work and data entry relating to patient care documentation. The CCMA demonstrates knowledge of the electronic medical system computer program and of proper documentation procedures. The CCMA demonstrates professionalism in speaking with patients and their families/caregivers, and with healthcare professionals and outside vendors. The CCMA takes responsibility for updating his/her knowledge of systems and procedures as these change. The CCMA supports the organizational vision and mission.

    EDUCATION AND QUALIFICATIONS:
    • Currently licensed in the State of Florida as a Licensed Practical Nurse (LPN), with a minimum of one (1) year of nursing experience
    • IV Certification is required
    • Bilingual preferred
    ESSENTIAL JOB RESPONSIBILITIES:
    • Enters all patient status changes in a timely manner.
    • Keeps team census spreadsheet up-to-date.
    • Maintains accurate disaster preparedness spreadsheet.
    • Maintains patient oxygen vendor usage spreadsheet.
    • Prepares daily status reports.
    • Enters care plan templates for interdisciplinary group meetings (IDG).
    • Prepares and distributes agenda for IDG.
    • Runs recertification alert report to ensure that patients' recertification are done in a timely manner.
    • Ensures that recertification paperwork is correct and complete, and brings it to IDG meeting.
    • Prepares sign-in-sheet for IDG meetings and makes sure that it is signed by all attendees.
    • Notifies all team members of patient status changes.
    • Notifies DME and pharmacies of patient status changes, as needed.
    • Notifies outside doctors of patient deaths.
    • Sends updates on patients when warranted.
    • Check patient folder for completion after patient discharge or death, and give to CCM for signature, and then to HIM.
    • Conducts clinical audits.
    • Distribute audit findings to those staff that have deficiencies.
    • Print out nursing schedule sheets each week for team nurses to fill in.
    • Run productivity reports upon request.
    • Special tasks: Each CCMA has special tasks that only he/she is responsible for. These may include:
    • Running Gulfside census reports.
    • Ordering office supplies.
    • Copying and distributing weekend and overnight triage reports.
    • Other items as assigned.
    • Maintains a professional relationship and serves as a liaison between all team disciplines.
    • Offers emotional support to the patient/family in accordance with the hospice philosophy and plan of care.
    • Observes Patients' Rights.
    • Maintains a positive attitude and caring spirit. Displays a "can do" attitude.
    • Comes to work as scheduled and consistently demonstrates dependability and punctuality.
    • Demonstrates honesty and integrity in all matters.
