- Provide primary medical management for patients through collaboration with a physician.
- Delivers patient care and patient education in a manner that is appropriate to patient's age, physical ability, educational level and culture.
- Conduct health assessments and evaluate the needs of patients in the primary care clinic.
- Conduct physical exams, order and/or perform laboratory and diagnostic tests, and make clinical judgments regarding results.
- Perform minor surgical procedures independently.
- Participate in the development of a comprehensive plan of care for each patient.
- Initiate appropriate consultation and referrals.
- Provide health information and teaching with emphasis on prevention and curative treatment.
- Act as primary care resource to the Pediatric Multidisciplinary Team
- Participate in educational activities with nurses, nurse practitioner, and medical students.
- Document History & Physical, assessment and treatment intervention in medical record.
- Participate in committees for quality of care, standards of care, policies and procedures, and other program development activities.
- Maintain current standards of practice and seek professional development.
- Participate in regularly scheduled clinical care, team and department meetings.
- Perform other duties assigned by physician and / or supervisor as required.
- Current certification and licensure as a Nurse Practitioner in the Commonwealth of Massachusetts
- Current certification and licensure as a Nurse Practitioner in the Commonwealth of Massachusetts.
- Ability to maintain patient confidentiality at all times as required by the health center and regulations.
- Experience working with neonatal, pediatric, adolescent, and young adults to age 21
- Experience in culturally diverse settings and passionate about community health.
- Experience working within a multidisciplinary team including skill in openness to input from staff at all levels of education and function within the health center.
- Ability to communicate clearly with patients and with staff.
- Strong leadership, interpersonal, teaching, and organizational skills.
- Willingness to work evenings and weekends as required
- Proficient Spanish, Portuguese and/or Cape Verdean desirable.
