    Full Time - Medication Focused Services Care Coordinator (Bachelor's Level)

    Hiring Range:
    $ $30.55 Per Hour

    The Medication Focused Services Care Coordinator will conduct routine, brief check-in phone calls or visits with clients who have completed their psychosocial treatment plans, are psychiatrically stable, and are currently only receiving psychiatric medications at KMHS.

    This position is full-time. Clients served include both youth and adults.


    Primary Responsibilities:

    • Provide regular contact (primarily by phone or Zoom) to clients receiving medication management as their only KMHS service. Update treatment plans annually via face-to-face or telehealth services.
    • Provide community resource information and supportive problem-solving to clients if requested by clients during check-in calls or visits.
    • Observe and evaluate client stability. If there are clinical indicators for a higher level of care (AOP team, Child & Family team), coordinate this transfer. Coordinate and participate in crisis intervention for clients if needed; determine if continuing in Level of Care 1.5 services is appropriate after the crisis is resolved.
    • Communicate promptly and effectively to Medical Providers and Medical Assistants regarding client care and any concerns voiced by clients regarding medications.
    • Oversee clients transitioning to community-based providers when a client states they are ready to leave KMHS services; communicate with medical assistants as this occurs; complete discharge summaries.
    • Document all clinical services thoroughly and accurately and submit all documentation promptly.
    • Meet weekly with Special Programs Director to receive clinical supervision. Provide feedback on any changes needed in this program to address client needs more effectively.
    • Pursue professional growth and development by seeking learning opportunities and integrating new learning into daily work practices.

    Minimum Qualifications:

    EDUCATION:
    Bachelor's Degree in Psychology, social services, or behavioral health field.

    Entry level (minimum 1 year of job-related experience)


    This position requires driving clients in a personal vehicle on behalf of the agency; therefore, the incumbent must successfully complete a motor vehicle history check, possess and maintain a current, valid driver's license in the state of Washington, and have reliable, insured transportation.


    LICENSURE:
    Agency Affiliated Counselor Registration.


    Preferred Qualifications:

    EXPERIENCE:
    Experienced (minimum 2 years of job-related experience)

    Experience in co-occurring disorders treatment, individual and group therapy, vocational services, or substance abuse treatment.

    The capacity to work well with children and families.
