    As a Licensed Practical Nurse (LPN) you will:

    • Provide in-home direct patient care following the provider's prescribed plan of care (and nursing scope of practice) while closely monitoring progress and recovery
    • Assess and accurately document patient conditions, reporting changes as necessary (vitals, blood/stool/urine, glucose, etc.)
    • Administer medication, insulin, and IV fluids while maintaining comprehensive documentation
    • Inspect and attend to wounds, change dressings, and assist with personal hygiene
    • Evaluate patients for additional injuries, infections, bedsores, etc., and provide suitable treatments
    • Work closely with providers, aides, and other healthcare professionals to ensure exceptional patient outcomes

    Requirements for a Licensed Practical Nurse (LPN) position with us:

    • Licensure: Current unrestricted license to practice as a Licensed Practical Nurse (LPN) in the state associated with this position
    • Current CPR/AED/BLS/First Aid certification
    • Access to reliable transportation to and from care sites and/or work locations
    • Experience with practical trach and/or ventilator procedures is preferred but not mandatory
