We are currently hiring a LEAD Coordinator, to work in our Care Coordination Services Office. As the LEAD Coordinator, you would be responsible for:

Address program vacancies or expansion by conducting interviews, obtaining references and completing other assigned tasks related to the hiring process.
Provide orientation and training to all new and existing staff as needed.
Facilitate general program oversight and development.
Monitor the oversight of all outreach and referral services provided through LEAD
Conduct initial screening and/or intake activities for diverted/referred participants
On Call phone rotation once every six weeks

LEAD Coordinator Minimum Salary: $58,500

LEAD Coordinator Maximum Salary: $58,500

LEAD Coordinator Schedule: Mon-Fri 8:30am-4:30pm (some evenings and weekends as required)

Qualifications:

Associates or Bachelors Degree in health, education or human services OR RN, LPN, CASAC
At least one year of supervisory experience of outreach, Case Management or case work
1- 3 years' experience with persons with: HIV/AIDS or other chronic conditions, a history of mental illness, homelessness, chemical dependence, and/or other populations of persons in need-one year of which must include supervisory experience/responsibilities
