    JOB DESCRIPTION SUMMARY

    The hospice aide, under a registered nurse's supervision, conducts assigned tasks to meet patient's needs and enhance comfort. Responsible for reporting and documenting care and observations.

    The hospice aide ensures quality, continuity, and safety in patient care assignments.

    ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

    The hospice aide's responsibilities include:

    • Performing personal care needs promptly after patient assignment, notifying Case Manager if unable to complete a task immediately.
    • Communicating positively with patients and families while maintaining confidentiality and ethical care.
    • Ensuring patient safety and using equipment appropriately.
    • Providing personal care such as baths, oral hygiene, and changing bed linen.
    • Assisting with dressing, undressing, and feeding patients.
    • Observing and caring for the patient's skin integrity and reporting any changes to the RN.
    • Assisting with ambulation, exercises, and medication reminders as permitted by regulations.
    • Providing assistance with mobility, positioning, and daily activities.
    • Offering support with basic procedures, housekeeping tasks, and respite care.
    • Keeping the patient's living area clean and following organizational procedures.
    • Participating in quality assessment and improvement activities.
    • Reporting any violations or neglect as required by regulations.

    JOB LIMITATIONS Hospice aides do not perform nursing duties or administer medication, following the Nurse Practice Act.

    POSITION QUALIFICATIONS

    • Completion of a certified nursing assistant course or equivalent training.
    • Current Healthcare Provider CPR certification.
    • COVID vaccination required due to job nature and multiple facility work.
    • High school diploma or equivalent.
    • Ability to read, follow instructions, and document care provided.
    • Proficient in hospice care with prior relevant experience preferred.
    • Basic computer skills for EMR documentation.
    • Minimum 18 years of age and licensed driver with insured vehicle.
