    Seven Healthcare is pleased to offer an exceptional opportunity for a Travel Case Management Registered Nurse in Dalton, Georgia. This assignment provides a weekly blended rate of $2,600, based on five 8‑hour day shifts. This role is perfect for an experienced RN Case Manager who excels in discharge planning, care coordination, utilization review, and patient advocacy across multidisciplinary clinical settings.

    Key Responsibilities of the Case Management RN

    Perform comprehensive patient assessments to determine discharge needs, resources, and risks
    Develop individualized care plans aligned with medical providers' recommendations
    Coordinate discharges to home health, SNF, rehab, outpatient therapy, or community resources
    Conduct utilization reviews and communicate with insurance providers as needed
    Educate patients and families regarding treatment plans, follow‑up care, and resource access
    Maintain timely and accurate EMR documentation for all case‑management activity
    Collaborate with physicians, therapists, social workers, and interdisciplinary teams
    Support smooth transitions of care and reduced readmission risk

    Requirements of the Case Management RN

    Active Georgia RN license
    Current BLS certification
    Minimum 1–2 years' recent Case Management / Care Coordination experience
    Strong assessment, communication, and documentation skills
