    Purpose: Responsible for Coordination of HOPWA housing program within the Rochester, Finger Lakes Region, and Southern Tier

    Detail of Key Job Responsibility Areas

    1. Coordinate HOPWA Rental Assistance program
    1. Manage housing intakes and unit inspections.
    2. Conduct annual recertification of households and units to determine eligibility
    3. Conduct monthly contacts of each household and document in electronic medical reporting system.
    4. Conduct assessments and develop service plans to assist households reach optimal independence
    5. Demonstrate developing knowledge of HIV/AIDS housing.
    2. Coordinate grant funded homeless prevention services
    1. Responsible for completing emergency assistance applications that assist HIV+ and non-HIV+ households from becoming homeless.
    2. Participate in the Rochester Area Task Force on AIDS, Housing Services Network, and other housing related activities in the community.
    3. Miscellaneous

    7. Ability to organize your day and work independently as well as be flexible and work in a team environment.
    8. Employees are accountable for meeting the performance standards of their departments and must participate as requested in compliance audits, process improvement and quality improvement plans.
    9. Other duties as assigned.
    Qualifications

    Associate degree in human services field preferred, high school diploma or equivalent required. Ability to speak Spanish and experience working with diverse populations preferred. Minimum two years of experience in residential services or advocacy, plus knowledge of affordable housing, subsidy programs, valid NYS driver license and reliable transportation required.

    Physical Requirements

    While performing the duties of this job the employee is required to stand, sit, walk, use hands to finger, handle, or feel; reach with hands and arms, talk, and hear. Occasionally the employee must stoop, bend and lift or move up to 25 lbs. Specific vision abilities required include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.
