Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts or transfer belts.
- Monitor patient observations by recording vital signs and noting changes in health status to ensure prompt response to any concerns.
- Support medication administration following medical documentation protocols while maintaining HIPAA compliance to protect patient privacy.
- Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments through tailored memory care techniques.
- Operate Electronic Health Record (EHR) and EMR systems accurately to document care activities and medical records efficiently.
- Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment for clients.
- Collaborate with healthcare professionals and family members to develop and implement personalized care plans aligned with each client's needs.
Requirements
- Proven experience in home health, long-term care, nursing homes, or assisted living environments.
- Knowledge of medical terminology, medication management, catheter care, hospice support, and dementia or Alzheimer's care.
- Certification in CPR and first aid is required; additional certifications such as HIPAA training are preferred.
- Ability to work independently while demonstrating excellent patient observation skills and compassion for working with individuals with developmental disabilities or disabilities of any kind.
- Experience operating medical equipment like Hoyer lifts or vital signs monitors is highly desirable.
- Strong communication skills to effectively document medical records and coordinate with healthcare teams.
- Valid driver's license and reliable transportation are often necessary for home visits; flexibility for live-in employment may be available.
