**Position:** Medical Assistant (Temp to Hire) OR CNA

**Salary:** $31-33/hr (Experience Dependent)

Need to be able to start by Monday, 5/6

**Schedule:** Hybrid, Mon-Fri (travel is required within the bay area in Daly City or South SF

**About the Role:**

As a Medical Assistant, you'll help members access vital medical and community resources to manage their health effectively. This includes assessing needs, connecting members with services, and addressing barriers to care. You'll also collaborate with community partners to fill resource gaps and ensure members receive comprehensive support.

In this role, you will be traveling to paitents homes on an appointment basis to assist them in accessing medical care. Mileage is reimbursed.

This is a hybrid role, where you will be working out in the field 80% and remotely 20%

**Responsibilities:**

Assess and document members' health needs

• Take vitals and to intake assessments.

Collaborate with licensed staff and community resources

Assist members in accessing medical and community services

Build rapport and communicate effectively with members and their support systems

Develop and document plans to address health disparities

Attend team meetings and provide updates on member progress

Fulfill reporting requirements as needed

You must have a valid driver's license and areliable car.
