    RN Care Coordinator - Grace Medical Center

    Our team is seeking a skilled RN Care Coordinator to provide individualized patient-centered goal-directed nursing care through the use of the nursing process and primary nursing principles. This role will involve coordinating care management for patient populations, including those identified by MDPCP, with the goal of improved health, reduced healthcare costs, and better coordination of care.

    Key Responsibilities:

    • Accepts care management referrals and utilizes evidence-based medicine, data analytics, and innovation in implementing care management principles.
    • Applies critical thinking skills and leadership skills in working with the team to meet patients and their families' needs.
    • Coordinates care to obtain desired health outcomes, improve self-care abilities, decrease cost of care, and provide extraordinary patient care.

    Requirements:

    • Graduate of an accredited school of nursing; bachelor's degree required.
    • Care/case management certified or knowledge of national care management standards and community resources a plus.
    • 3-5 years demonstrated proficiency in acute care nursing, knowledge, and skills.

    The estimated salary for this position is $65,000-$90,000 per year, depending on experience.
