Job Description:

Responsibilities: The Family Nurse Practitioner is responsible for providing primary care services, conducting in home health risk assessments, providing wellness education, supporting individual lifestyle changes, and helping individuals manage their chronic conditions. Maintaining a collaborative relationship with the PCE staff is a key ingredient to success in this position. Qualifications
• Minimum 1 years' experience practicing as a Family Nurse Practitioner within a primary care/family practice setting
• Electronic Medical Record (EMR) experience
• Current and unrestricted state licensure
