- Develop a personalized care plan based on nursing diagnosis.
- Conduct initial assessment of patients and families to identify home care requirements.
- Regularly reassess patients' nursing needs.
- Establish and adjust the care plan according to changes in patient status.
- Administer appropriate preventive and rehabilitative nursing procedures.
- Provide guidance to patients and families to meet nursing needs.
- Offer health care instructions based on assessments and plans.
- Identify discharge planning needs and implement them prior to patient discharge.
- Perform additional duties as needed.
- At least 2 years of RN experience.
