    Description

    THE ROLE
    The Director LTC RN provides clinical leadership, day-to-day clinical management and direction at the Chiniak Bay Elder House.

    Each are delivered in accordance with Providence philosophy, Federal and State regulatory standards ensuring the delivery of high-quality, cost-effective health care consistent with the mission of the Sisters of Providence and Providence Health & Services Alaska (PHSA).

    The Director LTC RN will actively participate as a member of Chiniak Bay Elder House Leadership Team and provide guidance about laws, rules and regulations pertaining to areas of work including but not limited to Utilization Review requirements as outlined in CMS Requirement of Participation (RoP) for long-term care and other regulatory guidance as applicable.

    The Director LTC RN must obtain an Alaska LTC Administrators license within 18 months of start date.


    ESSENTIAL FUNCTIONS

    COMMUNICATION:


    Demonstrates professional interpersonal and verbal skills when communicating with a wide variety of people from differing literacy levels, languages and cultures.

    Communicates effectively in a compassionate manner to residents, families, visitors, community members and other caregivers.

    Communicates regularly with caregivers and core leaders through rounding and other modalities.

    Exhibits the necessary written skills for effective communication and documentation.


    Regularly collaborates with the ECF Medical Director in regard to policies and procedures and medical staff issues, also communicates regularly with the Director of Administrative Services- LTC.

    Responds to requests and suggestions from resident and family councils.


    HUMAN RESOURCES:


    Uses judgment, effectively recommending and/or taking action related to interviewing, hiring, transferring promotions, evaluating performance, overstaff, layoffs, disciplinary action and employee complaints and concerns.

    Conduct and document employee performance on an annual basis and as needed.

    Uses appropriate interpersonal skills to guide others to accomplish objectives, facilitates team work, participation and cooperation.

    Delegates work so core leaders and caregivers have the necessary direction, authority and skills to complete assignments.

    Provides opportunity for core leader / caregiver growth as appropriate.


    QUALITY IMPROVEMENT:


    Assists in the annual development and implementation of the facility Quality Assurance Performance Improvement (QAPI) plan, Facility Assessments, QAPI self-assessments, and Performance Improvement Projects (PIPs).


    Addressing all aspects of clinical and business objectives and strategic plan in collaboration with the core leaders and the LTC Quality Assessment and Assurance team.

    In long-term care, directs the initiation and updates of a Quality Management program.

    Direct all LTC Clinical Teams through Annual Program Evaluations.

    Identify indicators and improvement processes utilizing the PDSA structures.

    Strives for and consistently achieves excellence in care provided to residents/patients in collaboration with multidisciplinary team.

    Seeks input from various customer groups in order to develop and maintain quality.

    Reports to the appropriate committees and boards are required.

    Collaborates with all appropriate departments using high reliability focus to promote optimal resident / patient centered care and customer service.

    Reviews survey results from caregivers, residents and families, directing the development of action plans using input in collaboration with core leaders.

    Implements, monitors and adjusts action plans to achieve caregivers, residents and family satisfaction.

    Maintains positive core leader /medical staff relationships sustaining an organizational climate resulting in the integration of physicians with the multidisciplinary team supporting positive outcomes.

    Represents PKIMC / Chiniak Bay Elder House to the community in an appropriate manner.

    Directs the development, modification and implementation of resident / patient centered care standards, policies, procedures and protocols.

    Establishes clear clinical practice expectations for providing evidence-based care.

    Demonstrates leadership principles fostering change in a culture of safety / high reliability using tones, tools and behaviors.


    TEAMWORK:
    Role models collaboration and partnership with physicians, caregivers, patients, residents, families and other departments.


    Exhibits the necessary skills to build effective teams by coaching, building trust, and training care givers to work collaboratively and cohesively with other caregivers in a variety of settings.

    Encourages positive interaction and if necessary, guides staff to work directly with other team members when resolving issues.

    Monitors key processes to identify and define the team's effectiveness.

    Support new caregivers in learning department routines.

    Fostering change and teamwork to ensure resident / patient centered care delivery, constructive team relationships, high reliability and growth.


    DEVELOPMENT AND ORGANIZING:


    Leads the multidisciplinary team core leaders to develop and implement work schedules /assignments fairly, equally, and in a manner which is consistent with caregiver learning, growth and development plans.


    Directs the development, implementation and maintenance of departmental policies and procedures in accordance with Federal, State and Municipal requirements to ensure safe and effective services.

    Participates in personnel functions including hiring, evaluations, disciplinary actions and terminations.

    Anticipates and prioritizes workflow and adjusts work assignments to prevent delays and meet workflow demands.

    Utilizes available resources to recognize errors/problems and takes corrective action and assists with problem resolutions.

    Seeks opportunities to promote PKIMC and Chiniak Bay Elder House.

    Reviews and approves multidisciplinary input of referrals for admission to the long-term care.

    Ensure a list of potential residents through Social Work core leader with the goal to maintain a full census.

    Direct care/services that recognize age/diversity specific needs/issues of customers served.


    BUDGET AND FINANCIAL RESOURCE:


    Assist in the completion and regularly monitoring the departmental operating and capital budgets under guidance of the Chief Financial Officer or designee.

    Anticipates and adjusts operating plans to meet facility goals.

    Involves core leaders and caregivers in budget planning.


    Coordinates and collects statistical and other information to support and develop current and/or new services while insuring cost effective processes and development plans are in place.

    Oversees service agreements and equipment purchases to support the departmental, facility and community needs.


    Ensures the financial strength of the facility by assisting in the development, monitoring and achievement of the facility's annual and long term financial operating and capital budgets, to achieve cost effectiveness, productivity and revenue maximization.


    Provides leadership and works collaboratively with core leaders and caregivers to evaluate new opportunities and to develop programs and services that improve the organization's market position and meet community needs.

    PURCHASING:


    Ensures an effective purchasing process is maintained by caregiver requisitioning necessary products and supplies for personnel, determining necessary quality/quantity requirements, selecting suppliers according to current contracting, arranging for deliveries, following up on orders, dispensing of equipment and other items when necessary.

    Directs the development and/or maintenance of department records keeping system, regarding par levels of department supplies.

    Submit invoices for approval and processing, reconciles P-card and receipts, submitting in a timely manner.


    COMPLIANCE:


    Leads multidisciplinary team to ensure high quality, cost effective resident / patient centered care services, which meet or surpass survey standards.

    Maintain and guide the implementation of organization policies and procedure in compliance with State, Federal and other regulatory guidelines.

    Participates in mock surveys, develops plans of correction for deficient areas.

    Assure adequate preparation for and participate in regulatory and other compliance surveys.


    PROFESSIONAL DEVELOPMENT:
    Assumes responsibility for obtaining information and expanding job knowledge and competency that reflects current practice.

    Role models personal development and education.

    Evaluates own practice in relation to professional practice standards and guidelines, relevant statutes, rules and regulations.

    PATIENT/


    RESIDENT CARE:


    Leads team to ensure resident / patient care is in compliance with State and Federal regulations as well as PKIMC and Long-term care policies and procedures.

    Recognizes age and diversity specific needs and issues of our residents /patients and promotes appropriate response.

    Participates in after hour call to support of the Long-term care and after hour's emergencies as they arise. Acts as a resident / patient care advocate to ensure that the resident's / patient's wishes are heard.

    Makes sure that the resident / patient is receiving only those services that are effective and safe.


    ENSURING QUALITY OF CARE:
    Coordinates with the multidisciplinary team to establish and monitor each resident's overall plan of care including discharge planning.

    Promotes input from all personnel, departments, and services to plan individualized care.

    Adheres to the philosophy of Resident's Rights and ascertains needs for additional or modified services.

    Oversees and reviews the completion of the Minimum Data Set (MDS) and the Quality Management Data for Long-term care.


    INFECTION CONTROL:
    Leads team for infection prevention oversight to ensure compliance with CDC and OSHA standards; State and Federal Regulations.


    Ensures the Infection Prevention Specialist coordinates staff education related to infection control practices, performs surveillance of infections, initiates and conducts epidemiology investigations of unusual trends or outbreaks of communicable diseases.

    Participates in environmental rounds.

    MDS/


    RAI:


    Leads and directs the MDS/RAI, Care Area Assessment, care plan process in support of MDS Coordinator to meet State, Federal and quality requirements.

    Supervise and collaborates with MDS Coordinator to ensure nurses complete nursing assessments a timely for MDS.

    Understands MDS criteria.

    Ensure nursing coverage to complete resident MDS/RAI process completion in accordance to prescribed times.


    INVESTIGATIONS:


    Assists in investigation of resident, family, and employee concern such as but not limited to Unusual Occurrence Reports (UORs), Employee Injury Reports (EIRs), resident concerns or 1:1 meetings.

    Identifies root cause and develops correct action plans.

    Prepares and submits necessary and/or required facility and/or state reports according to state regulations on reporting or department guidelines.

    Works with Grievance official/officer and Administrator

    Directs and supports the implementation of evidence based care/services that recognize age/diversity specific needs/issues of customers served.

    Demonstrates awareness of resident / patient rights in all aspects of care, treating each resident, patient and co-worker with respect and dignity.

    Respects the rights of patients, residents and caregivers; provides privacy and maintains confidentiality.


    Leads collaboratively with Executive Leadership team, Servant Leadership team, Department managers / core leaders and caregivers to maintain a safe and resident centered environment for each resident.

    Maintains awareness of accident prevention practices and provides a safe environment for all.

    Completes Competency Plan for assigned job and department.


    QUALIFICATIONS
    Bachelor's Degree in Nursing degree/diploma required upon hire. Bachelor's degree in nursing within 3 years of hire. LTC Administrators License required within 18 months of hire

    Master's Degree (preferred)

    Licensed as a Nursing Home Administrator in the State of Alaska within 18 months of hire or when meeting eligibility for exam

    5 years of progressive Nursing Management experience

    Effective skills in management and leadership, verbal and written skills, and special interest in long-term care

    Knowledge of the principles and theory underlying Long-Term Care practice and ability to maintain legal records.
