Job summary
The Medical Social Work Associate provides crisis interventioninformation referrals and discharge planning for patients and their families.
Serves as an expert and liaison to community resources. Provides coordination of complex discharge planning needs
and referrals to appropriate support services.
- Masters Degree from a program accredited by the Council on Social Work Education
- Two years experience in a Health Care setting
- Knowledge of Medicare MediCal State and Federal regulations.
