    The Screening & Prevention program is designed to provide personalized oncology-focused screening services and cancer prevention education to our community. The Screening and Prevention Coordinator will help coordinate cancer-risk surveillance and management recommendations as well as any accompanied referrals; provide accurate and up-to-date information about cancer risk and risk reduction strategies; assist in the screening and early detection of cancer; provide information and education to patients regarding related health issues; serve as an expert resource on high-risk cancers and genetics for both patients and clinicians; and work in partnership and collaboration with other members of the health care team, both inside and outside Baptist Cancer Institute. The Screening and Prevention Coordinator ensures timely access to healthcare services and provides patient information and education, as needed, to ensure the best patient healthcare experience possible. The position will also support and help execute specific Baptist Cancer Institute initiatives and accreditation support as directed by the Cancer Support Services Senior Manager and Screening & Prevention Manager.

  • Provide comprehensive risk assessment and screening recommendations of individual patients.
  • Review of patient's medical history, patient's family history, ensuring the accuracy of patient entered electronic data as well as individual patient pedigree.
  • Manages Screening & Prevention medical management recommendations for patients identified with elevated cancer risk.
  • Verifies health insurance eligibility and understands copays, co-insurance and deductibles. Obtains Pre-authorizations.
  • Responsible for care coordination of medical management changes and navigating patient across multiple service lines to ensure timely follow-up of diagnostic services.
  • Accountable for high-risk patient care continuum from identifying patients at risk to the delivery of personalized prevention, screening & medical management updates.
  • Provide complete oversight and management of all related Electronic Health Record systems as well as program volumes and metrics to leadership as requested.
  • Clinician timely genetic education and is a resource to our system, providers and community for cancer prevention resources and education.

  • Assist in other duties as assigned to support the operational needs of the department and organization.
  • Associate Degree in Nursing or Nursing Degree is required.
  • Four (4) years' experience in clinical-based nursing or clinical case management. Two (2) years' experience in nurse navigation or care coordination preferred.
  • Current RN licensure in the state of Florida or eligible compact licensure.
  • Demonstrated skills in the areas of negotiation, communication (verbal and written), conflict, interdisciplinary collaboration, management, creative problem solving, and critical thinking. Knowledge of healthcare financing, community and organizational resources, patient care processes, and data analysis.
  • Excellent verbal and written communication skills required.
  • Demonstrates flexibility via an ability to adapt to changing priorities and regulations.
  • Basic computer skills.
