- Works in collaboration and continuous partnership with chronically ill or "high risk" patients and their family/caregivers, providers, facilities, clinics/hospitals, staff and community resources in a team approach.
- Assess phone calls with the ability to determine the urgency of patient, family, or facility request; if uncertain or require a level of expertise beyond LPN scope, check with RN or provider.
- Handle patient, family and facility advice calls, discuss test results, answer questions; if uncertain or require a level of expertise beyond LPN scope, check with RN or provider.
- Provides optimal care to patients with regulatory guidelines and LPN scope of practice.
- Assist with the identification of "high risk" patients and flag in the EMR.
- Medication management, including identifying potential medication issues, answering questions on side effects and medication conflict.
- Complete any prior authorizations for medications, DME, procedures, etc.
- Initiate/assist with DME, Home Health, home infusion/ PT/OT orders as directed.
- Completes and locks documentation on the same day as the contact/communication occurs as per the Medical Records Documentation policy.
- Record accurate and concise documentation pertaining to the patient history and phone conversation.
- Enter and follow up with orders from providers
- Prep patient chart(s) for providers
- Monitor Athena clinical inboxes for providers
- Track monitor and schedule patient labs, tests or procedures ordered by provider.
- Various clinical and administrative tasks.
- Participation in IDG, team and organizational meetings as required.
- Demonstrates preparedness for IDG meetings and provides IDG updates to a teammate prior to any planned vacations.
Qualifications
Qualifications
Highschool Diploma or equivalent
Certified Nursing Assistant (CNA) or Certified Medical Assistant (MA) (Preferred)
2+ years of experience in a healthcare setting (preferred)
Familiarity with electronic medical records (EMRs)
Strong clinical skills
Strong interpersonal communication, organization and computer skills
Familiarity with principles related to end of life
Certifications, Licenses, Registrations Required
Current certification in North Dakota and Minnesota (if necessary)
Current driver's license and auto insurance

Our house calls program is seeking a Medical Assistant (MA) or Certified Nursing Assistant (CNA) to provide administrative support for our team of medical professionals who perform house calls for patients. The MA/CNA will work in an office setting and will be responsible for maintaining patient records, scheduling appointments, preparing documentation, and providing general administrative assistance to the team.
