    Description

    We have an excellent opening for a Permanency Specialist responsible for individualized permanency planning, which includes the delivery of Family Finding services, targeted recruitment for adoption, provision of post-adoption services and/or post-guardianship services for youth and families served by Hillside at our Rochester, NY location.

    Part time position (20 hours) Monday - Friday, 1pm-5pm but flexible schedule to accommodate family needs/availability. Some evenings and weekends are required.


    The person in this position works with counties, community partners, clinicians, care coordinators and other care providers to identify youth in need of permanency planning and/or permanency maintenance and will locate and engage family members and other significant adults to build a lifetime network of supportive caring adults for each youth and family.


    Responsibilities:

    • Assess and address the permanency needs of children/youth, and/or partnering with families who want to provide permanency for children/youth.
    • Provide support for youth and families enrolled in services within the Institute for Family Connections (IFC).
    • Create a strength-based permanency plan that includes home visits and assessments, identifying and coordinating potential permanency resources and other natural supports, recognizing gaps and needs, assessing results and providing on-going communication with the youth's treatment team.
    • Actively participant in agency Permanency Roundtables and serve as a permanency consultant to multidisciplinary treatment teams.


    Requirements:


    Education:

    • Associate's degree in a Human Services related field required.

    Experience:

    • 3 years' experience in a human services or a related field required

    Requirements:


    • Unrestricted, valid NYS driver's license with a clean driving record and minimum insurance coverage that meets agency standards.
    • CPR certification required to obtain within 60 days of hire.
    $15.74 Minimum Rate per hour, $23.60 Maximum Rate per hour, based on experience.
