~ Adoption assistance
~ Hospice patient identification, follow-up, and management in the hospital
Manage Hospice referrals for assigned area, which may come from several sources including but not limited to: Determine Hospice level of care needs for each referral
Call/ conduct visits with patients/caregivers to explain and discuss Hospice services
Process appropriate documents including physician orders/certification
Communicate with Hospice Supervisor/Manager regarding status of referral to facilitate continued admission process and transition of care as needed by the patient
We are looking for compassionate Hospice Admissions Registered Nurse with:
Associate degree or higher from an accredited School of Nursing
Two years of RN experience, hospice experience preferred

The low end of the range includes compensation related to salary and first year success bonus.
