Mental Health Clinical Case Manager - San Mateo

They conduct IOP orientations for new patients introducing program structure expectations and resources use motivational interviewing and psychoeducation to enhance retention reduce early drop-out sustain engagement lead discharge planning beginning at admission ensuring linkage to outpatient providers community resources and payor-approved step-down services document collateral contacts discharge coordination and case management activities in compliance with CMS and commercial payor standards ensure IPC is properly captured for utilization review and reimbursement audits co-facilitate psychoeducation or skills-based groups provide clinical coverage for group sessions or therapeutic check-ins during staff absences within scope of licensure participate in weekly case conferences contributing psychosocial systems-level insights to treatment planning serve as liaison between therapists psychiatrists coordinators centralized navigation ensuring seamless care track engagement discharge readmission metrics report trends to the Clinic IOP Director for quality-improvement initiatives implement corrective actions to improve retention discharge quality community integration assist updating safety plans activating crisis escalation pathways as needed provide documentation meeting licensure accreditation payor requirements
3-5 years experience working as a Clinical Case Manager as a Licensed practitioner current clinical licensure state California must physically reside state California strong working knowledge evidence-based treatments mood anxiety trauma proficiency telehealth platforms remote clinical work excellent interpersonal communication documentation skills intact Licensed Marriage Family Therapist MFT or Licensed Clinical Social Worker LCSW
