    *Job description*Advanced Home Care and Hospice of Northern Idaho is looking for an RN case manager to join our growing home health team in Lewiston Candidates will be friendly, positive and focused on excellence AHC Home Health is an agency where the focus is on delivering 5-star services and exceptional patient outcomes.

    *Job Responsibilities:
    • Perform admission, discharge, and routine home health visits.
    • Coordinate & communicate patient care plans with interdisciplinary team.
    • Document all phone calls and visits using our EMR.
