    Bilingual Case Manager

    Part Time (20 hours)

    Keystone Health is seeking a part time Bilingual Case Manager to join the team.

    The Bilingual Case Manager the Case Manager is responsible for assessing and addressing the social determinants of health (SDOH) for migrant and seasonal farm workers. This role also involves providing health education to migrant and seasonal farm workers at farm worker homes, farms, labor camps, and other outreach settings.

    Duties include screens, interviews, and assesses farm workers for SDOH disparities. Appropriately refers and enrolls patients to these services. Arrange follow up for patients for provider-placed consultation referrals. Assist patients with the completion of hospital-based charity care programs applications and enrollment. Collaborates with outreach coordinators at monthly outreach coordinator meetings. During clinic hours, covers front desk operations including greeting patients, check-in, privacy notices, and payment collection. Attends weekly regional staff meetings. As available, accompanies multidisciplinary teams on outreach trips to farm worker housing. Acts as an in-the-field social worker as SDOH questions arise. Assists with registration and screening of farm workers as needed. Provides non-judgmental, linguistically, and culturally appropriate health education to migrant and seasonal farm workers. This education includes but is not limited to the following topics: Chronic disease prevention through diet and exercise, diabetes and hypertension, alcohol and drug abuse, mental health and depression, prevention and recognition of sexually transmitted infection, general recommendations for age-appropriate immunizations, and cancer screening tests. Registers farm workers eligible to participate in the Keystone Agricultural Worker Program according to Program guidelines. Assists the regional coordinator in identifying farms employing migrant and seasonal farm workers. Offers counseling services as licensure permits.

    Qualifications: Bachelor's Degree in Social Work strongly preferred or degree in relevant field with commensurate experience in social work.

    Ability to read, write and speak English and Spanish proficiently is required.
