    About the Job

    Job Responsibilities

    • Deliver post-acute care as an LPN
    • Monitor patients' physical and mental wellbeing and track vital signs
    • Perform intakes of residents, document their health concerns, and review medical records
    • Administer medications, maintain medical supplies, and contribute to a safe environment
    • Assist medical staff with orientation, training, and procedures like IV set up, vaccinations, and phlebotomies

    Qualifications and Skills

    • Current Connecticut state license to practice as a Licensed Practical Nurse (LPN)
    • Completion of approved LPN educational program
    • High school diploma (or equivalent)
    • At least 1 year of experience (LPN or related role)
