    Are you a motivated and licensed Physical Therapist looking to make a real impact in the lives of patients? We have an exciting opportunity for you to provide in-home physical therapy services to the Grand Junction community.

    Here's what you can expect in this rewarding role:


    Clinical Responsibilities:

    • Perform physical therapy visits, including evaluations, Start of Care (SOC), reassessments, recertifications, and discharge visits.
    • Develop and implement individualized physical therapy plans of care based on patient needs and physician orders.
    • Provide skilled interventions such as:
    • Gait training
    • Balance training
    • Therapeutic exercise
    • Mobility training
    • Fall prevention
    • Educate patients and caregivers on treatment plans, home exercise programs, and safety strategies.
    • Communicate patient progress and concerns to the interdisciplinary care team.
    • Collaborate in the clinical oversight of Physical Therapist Assistants (PTAs) when applicable.

    Documentation & Systems:

    • Complete all required documentation accurately and timely in Homecare Homebase (HCHB).
    • Ensure documentation complies with Medicare Conditions of Participation and agency standards.
    • Maintain compliance with HIPAA and all applicable privacy and security requirements.

    Care Coordination:

    • Collaborate with agency clinical staff, including RNs, PTAs, OTs, STs, and leadership.
    • Participate in care coordination relating to patient needs and visit completion.
    • Provide clinical oversight of PTAs when applicable and permitted.

    Qualifications:
    -
    Required:

    • Current, unrestricted Colorado Physical Therapist license.
    • Graduation from an accredited Physical Therapy program.
    • Valid driver's license and reliable transportation.
    • Ability to practice independently in a homebased setting.
    • Proficiency or willingness to learn Homecare Homebase (HCHB).
    -
    Preferred:

    • Home health experience.
    • Familiarity with Medicare documentation and home health regulations.
    • Experience working with geriatric and medically complex populations.

    Practice Environment:

    • Patientcentered, inhome care.
    • Oneonone visits.
    • Supportive clinical and administrative coordination.
    • Missiondriven home health services in the Grand Junction community.

    Shift Information:

    • Monday
    • Friday 8 am5 pm.

    Minimum Years of Experience:

    • Less than 1 year.
