    LEAD DIRECT SUPPORT PROFESSIONAL (LEAD DSP)

    Center for Developmentally Disabled (CDD) is a private, not-for-profit corporation founded in 1972 serving adults and children with developmental and intellectual disabilities within the greater Kansas City, MO area. CDD provides residential care services, employment services, day habilitation to adults, and a private school for students who are referred through the Kansas City Missouri area.

    NOTICE to internal candidates:

    For those applying for a promotion to a group home lead, we must have the completed internal application, and a Class A, B, C. or E class license which allows you to drive CDD's van/vehicle to transport residents. A Driver review check list is also required.

    For those apply for a promotion to an ISL lead, we just have the completed internal application, a Class E or F license, a driver review check list, and proof of personal auto insurance coverage to transport residents.

    Essential Functions

    Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions

    • Completes documentation for individual Person Centered Plan as required by plan;
    • Communicates daily with all staff to provide consistency in addressing each individual's needs verbally and in the daily communication logs;
    • Monitors personal appearance of individuals;
    • Attends Annual Person Centered Plan meetings with individuals;
    • Monitors individuals' Person Center Plans to ensure accurate implementation, on-going progress, and timely and accurate required documentation;
    • Assists with completing domestic duties when an individual refuse, is unable, or does not complete the duties in a satisfactory manner;
    • Transport individuals daily or as needed;
    • Assists with planning activities for individuals;
    • Assists with monthly reviews by completing the family/friends and community involvement sections, and with acquiring signatures from individuals;
    • Completes necessary documentation needed for the Annual Person Centered Plan packets;
    • Assist individuals in taking medication as prescribed by physician;
    • Attends staff meetings and in-service trainings as scheduled or as needed;
    • Maintains effective working relationships with supervisor and peers;
    • Checks homes and/or apartments for overall maintenance needs and documents needed repairs;
    • Writes and submits Event Reports;
    • Attend and participates in Individual Rights and Lead Direct Support Professional meetings;
    • Provides companionship, emotional support, crisis intervention, and behavioral counseling to individuals;

    ISL Lead Duties

    • Schedules and accompanies individuals to doctor appointments;
    • Checks in monthly cycle medications;
    • Checks MARS on a daily basis and provides follow up information to the Manager;
    • Conducts monthly meetings with the individuals at the home;
    • Prepares monthly menu;
    • Completes grocery shopping;
    • Assist with completing the monthly Manager's report

    Official Duties in the Absence of the Manager:

    • Obtains medical treatment for illnesses or injuries in a timely manner;
    • Is responsible for all medical appointments by scheduling the transporting individuals to medical appointments (physicals, dental, eye exams, lab work, etc...);
    • Ensures that all medication changes are clearly documented in CRB's, Physicians Orders and MAR;
    • Sets up MAR and checks daily for accuracy, keeping all staff, the pharmacy, the administrative office, and KCRO or EITAS informed of the changes;
    • Ensures that all medical appointments are recorded accurately in each person's case record;
    • Maintains files and case records; corrects and responds to items noted during the monthly reviews, Quality Assurance Committee and Case Record Review Committee meeting minutes, and by completing all filing on an as-needed basis;
    • Schedules regular emergency drills and completes reports as required;
    • Oversees individual money expenditures and maintains required records;
    • Assigns duties to staff;
    • Has on-call responsibility, altering schedule as necessary to meet individual needs;
    • Requests weekly household and individual petty cash funds; plans and maintains menus; orders and purchases food and household supplies; replaces worn items and appliances assuring adequate supplies and equipment are available to operate the facility;
    • Assists with completing domestic duties when a individual refuses, is unable, or does not complete satisfactorily;
    • Schedules maintenance and oversees the site implementation of the agency's Physical Facilities and Safety Policies; inspects all areas of the facility daily, maintaining adequate housecleaning standards and notifies the office of needed repairs;
    • Ensures safe and continuous operation of assigned vehicles, i.e. schedules maintenance and repairs as needed, reports repairs, accidents, and traffic violations;
    • Schedules vans for transportation needs.

    Supervisory Responsibility

    This position as no supervisory responsibility

    Work Environment

    This job operates in a group home type setting and will require exposure to human elements such as (blood, urine, etc.). This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

    Physical Demands

    The employee is occasionally required to sit, stoop, kneel, climb or bend. The employee must constantly lift and/or move up to 10 pounds, frequently lift and/or move 20 pounds and occasionally lift and/or move up to 50 pounds. Also requires stepping up into commercial passenger vans.

    Position Type and Expected Hours of Work

    This is a full time position. Days and hours of work consist of a combination of 4-5 days per week from 32 hours to 40 hours per week depending on shift.

    Travel

    Driving throughout the metropolitan area daily or as needed

    Required Education and Experience
