    Description

    The Pharmacist is responsible for evaluating and processing medication orders, compounding and dispensing medication, and reviewing evaluating recommending and managing drug therapy.

    The Pharmacist at the Centra Pearson Cancer Center provide comprehensive pharmaceutical care to oncology patients.

    This position utilizes information systems and medical records to obtain patient information and perform the appropriate clinical pharmacy functions, promotes rational drug therapy, performs patient consultations, and supports oncology clinical programs.

    This position serves as a resource for medical staff.

    Responsibilities

    Evaluates medication orders for appropriate indication, dose, route, strength, dosage form, and frequency of administration in relation to the medication history and current medication orders

    Utilizes pharmacy information and drug dispensing systems. Troubleshoots problems and manages system interruptions

    Dispenses medication accurately and with appropriate labeling

    Knowledgeable of pharmaceutical compounding. Compounds oral, topical, and intravenous products. Compounds parenteral nutrition, cancer chemotherapy, neonatal and other special products as assigned. Manufactures products using a laminar airflow hood, biological safety cabinet, compounding aseptic isolator, or clean room as assigned. Follows USP standards

    Applies knowledge of clinical fundamentals. Reviews the medical record and makes appropriate recommendations for drug therapy

    Makes dose recommendations after applying knowledge of pharmacokinetics

    Fills outpatient prescriptions and creates required documents

    Provides timely and accurate drug information to prescribers and nursing personnel

    Knowledgeable of drug procurement procedures. Secures emergency supplies of medication when needed. Assures proper drug storage. Assigns appropriate beyond use dates

    Recognizes medication errors and controlled substance losses, files complete reports, analyzes causes, evaluates alternative strategies for avoidance, and makes recommendations for improvement

    Supervises pharmacy technicians

    Coaches and trains new personnel, residents, interns, and other students

    For pharmacist positions in Oncology, updates chemotherapy regimen and formulary

    Other duties as assigned

    Qualifications


    Required Education:
    Bachelors Degree in Pharmacy

    Preferred Education:
    Doctoral Degree in Pharmacy (PharmD)

    Preferred Experience:
    Hospital pharmacy practice experience. Hospital Residency (Post Graduate Year - PGY1). Oncology experience for Pharmacist positions with Oncology

    Required Certifications and Licensures:
    Licensed to practice pharmacy in the state of Virginia
