    Description

    Job Summary

    Under the direction of a Registered Nurse (RN), this role provides continuous visual monitoring and surveillance/observation of assigned patient(s). The primary responsibility is to verbally redirect the patient from engaging in at-risk behaviors and summons the nursing staff if the patient requires assistance.

    Key Responsibilities:

    • Utilize observational skills to provide continuous visual monitoring, support, and observation of assigned patients. Coordinate relief when leaving the patient's room to maintain constant patient observation.
    • Maintain visual patient observation at all times. Verbally redirect patient as needed to ensure patient safety. Summon nursing staff if the patient requires assistance with activity or ADLs.
    • Provide patient explanations as necessary. Escalate to clinical staff on unit when necessary.
    • If patient appears to pose a threat to themselves or others, escalate to clinical staff on the unit.
    • Ensure patient's primary language, if other than English, is assigned in system by nursing staff.
    • Participate in collaborative identification and reporting of patient safety issues and ensure patient environmental safety. Seek assistance with removal of any potential safety hazard from room and report findings to charge nurse. Escalate any patient safety hazard identified in room to clinical staff unit.
    • Participate in safe patient hand-off at break or change of shift.

    Requirements:

    Education:

    • High School Diploma Preferred: or equivalent

    Experience:

    • 1 year Preferred: of patient care experience

    Credentials:

    • Basic Life Support Required

    Skills:

    • Able to communicate clearly and effectively with patients.
    • Ability to communicate with hospital staff and respond quickly to patient behavioral changes.
    • Ability to perform duties requiring prolonged and/or extensive sitting, standing, and walking.
    • Ability to use discretion and courtesy when working with patients, visitors, and staff.
    • Efficiency in multitasking, and ability to prioritize simultaneous situations.
    • Some proficiency with computer skills.

    Working Conditions:

    Lifting:

    • Patient handling no greater than 35 lbs. without the use of assistive equipment and/or devices (NIOSH).
    • Fine motor skills to be able to grasp and control medical equipment and perform precise procedures.
    • Push/Pull: Frequently up to 45 lbs. force (i.e., WOW, medical carts).
    • Perform hands-on CPR at least 20 minutes using 100-125 lbs. of force (National Assoc. of EMS Physicians and AHA) Frequency should be seldom or occasionally.
    • Ability to move around area with frequent sitting.
    • Bending/stooping/squatting/reaching/kneeling frequently.
    • Lifting (non-patient) up to 20 lbs. occasionally and 5 lbs. frequently.

    Environmental Conditions:

    • Exposure to biohazard, body fluids, and airborne particles.
    • Must be able to complete tasks in a noisy environment.

    Mental/Visual:

    • Vision and hearing required within normal limits (glasses, contacts, hearing aids permitted).
    • Ability to communicate and exchange accurate information.
