    Description
    Join our team at Healthy Living of Bend and make a difference in the lives of our patients and their families. We are currently seeking a passionate and dedicated Per Diem Medical Social Worker (MSW) to provide support and care in the comfort of patients' homes.


    Location:

    Bend, ORSalary:
    Starting at $38/ hour, DOE Qualifications:
    • Graduate of a bachelor's program in social work accredited by the Council on Social Work Education.
    Master's or doctoral degree in social work is a plus.
    • Licensed as a licensed clinical social worker.
    • Minimum of one (1) year of experience in healthcare, with preference given to candidates with experience in home health care.
    • Excellent verbal and written communication skills, as well as strong organizational abilities.
    • Current CPR certification.
    • Must be a licensed driver with access to a reliable, insured vehicle in good working order.
    Responsibilities:
    • Assess the psychosocial status of patients in relation to their illness and environment.
    • Conduct social evaluations and develop intervention plans based on assessment findings.
    • Maintain accurate clinical records for all patients referred to social work services.
    • Provide information and referral services to patients and families/caregivers regarding practical and environmental needs.
    • Serve as a liaison between patients/families/caregivers and community agencies.
