    Description
    Facilitates, communicates and collaborates with the RN to identify and meet the physical and emotional needs of the patient. Assists with the nursing process to promote/restore optimal health and well-being of the patient. Assumes personal accountability for organizational mission and core values. Demonstrates accountability for the quality of own nursing practice. Performs a wide variety of patient care activities and accommodative services as assigned.

    • Performs accurate observation & data collection.
    • Perform IV therapy or venous blood draws .
    • Appropriate communication of patient findings to the RN.
    • With patient and significant others, contributes to & suggests resources in the formation and revision of POC.
    • With patient and significant others, assists RN in development of discharge plan of care.
    • Organizes time and priorities appropriately.
    • Differentiates priority of risk & urgency.
    • Uses critical thinking to implement plan of care.
    • Communicates plan of care to patient and significant others and documents patient outcomes.
    • Directs staff in implementation of plan of care.
    • Practices nursing in accordance with appropriate guidelines of practice.
    • Continuously evaluates and assists team in revisions of the plan of care.
    • Accountable for the nursing process, responds to QI monitoring.
    • Attends staff meeting, including training meetings.
    • Attained 10 CEU'sAdheres to department/organizational policies and guidelines

    Minimum Experience Requirements:
    Two years of recent LPN experience preferred

    Float Requirements:

    • Floating may be required to any Centura location within sixty (60) miles of the original assignment location or Centura identified "float zone".
    -Float assignments may include duties outside of original assignment job requirements (including skill set) in accordance with Centura policy.
