Job Description:
Coordinates the total Plan of Care for the patient in the Home Care setting assessing, planning, implementing and evaluating.

Provides leadership for the Health Care team by working cooperatively with other team members in maintaining standards of professional nursing practice in the Home Care setting.

Minimum or Preferred Qualifications:
Graduate of accredited School of Registered Nursing. One (1) year medical/surgical experience as a RN. Current licensure to practice as a Registered Nurse in NYS. Valid NYS drivers license.

8:00AM-4:30PM with evening (12:00PM-8:30PM), Week-end and Holiday rotation.


Salary Range:
$60 per revisit; $170 per start of Care; $25 an hour for edu/office time
