Lakewood Health System is seeking a compassionate and experienced Care Management/Palliative Care Social Worker to join our team.
We will collaborate with healthcare providers to support patients through their chronic care journey and provide social services that foster their well-being.
Responsibilities
- Develop individualized care plans in collaboration with physicians and interdisciplinary teams.
- Assess patients' needs and provide follow-up as appropriate.
- Serve as a liaison between patients, families, clinics, and multidisciplinary teams.
