Job summary
The PostRelease Therapist supports participants on their healing journey working alongside a comprehensive care team you will employ our healingfirst approach and help participants create and achieve the goals in their treatment plan facilitating excellent participant programming conducting impactful weekly 11 and group sessions with participants utilizing Cognitive Behavioral Therapy and Motivational Interviewing approaches to support participants in achieving their goals providing frequent supportive communications to participants throughout postrelease phases updating participant objectives and goals in their treatment plans using assessments to confirm treatment plan goals and progress upholding the quality and fidelity of Concordance Reentry Model curriculum when delivering 11and group sessions preparing for sessions in advance to maximize effectiveness of interactions evaluating participant progress and support needed with the use of assessments other evaluations as needed communicating with multidisciplinary care team documenting sessions including noteworthy participant interactions in electronic record system working collaboratively with participant care teams providing input on progress flagging holistic participant needs participating in internal meetings behavioral health wellness team meetings all team meetings supervisor meeting training sessionsotherwise assisting staff members occasionally helping set up for Center events or move classroom around.
