The Chronic Care Management Nurse works as part of the clinic provider team, assists the provision of primary health care, provides direct patient services, and performs specifically designated independent procedures.

The Chronic Care Nurse will plan and coordinate self-care management as part of a multidisciplinary team.

The Chronic Care Management Nurse will develop a comprehensive service delivery system aimed at meeting the needs of the patients/clients.

Remote Patient Monitoring of patients also required. Applicants must be willing to travel throughout service area, and must have a current RN/LPN license and CPR.
