Qualifications

Required Skills:

• Current clinical licensure strongly preferred.
• One year recent relevant experience required. Acute care setting preferred.

Responsibilities

Collaborates with physicians and team members to develop care plans for patients, ensures progress towards desired outcomes, resolves care barriers, and actively participates in discharge planning while incorporating the hospital's mission into daily practice.

Posted Total Compensation

The wage range for this role considers factors such as skill sets, experience, education, licensure, and organizational needs. Compensation decisions are made based on individual circumstances. The estimated range for this role is between USD $34.06/Hr to USD $45.00/Hr.
