    We are seeking a compassionate, dedicated Inpatient Transplant Coordinator to execute the following job duties with our inpatient liver transplant population.

    Essential Duties:

    • Responsible for the planning and coordination of all aspects of liver transplant patients' care from referral to post transplant.
    • Serves as the primary coordinator for the interdisciplinary team, including physicians, nurses, social workers, dieticians, pharmacists, financial analysts, case managers, and support staff
    • Develops, coordinates, implements, and evaluates educational programs/projects for patients, personnel and the community
    • Collects/documents data to meet clinical, financial, and regulatory requirements
    • Participates in the development of guidelines, protocols, pathways and other performance improvement activities
    • Participates in research activities
    • Serves as a patient advocate throughout the transplant/surgical process
    • Represents the Medical Center at regional and national meeting/conferences

    Education:

    Graduate of an accredited nursing program required

    BSN required

    License/Certifications:

    • Current, unrestricted California RN License required
    • BLS from the American Heart Association required
    • Specialty or Transplant certification required or willing to obtain within one year from start date

    Experience:

    • A minimum of 3 years Acute Care or Critical Care experience required
    • Experience as a Transplant Coordinator preferred
