    Job Title: Registered Nurse

    Summary:

    We are seeking a compassionate and skilled Registered Nurse to join our hospice program in Virginia. As a Registered Nurse, you will play a vital role in ensuring the comfort, dignity, and well-being of our patients.

    Responsibilities:

    • Conduct nursing evaluation visits, develop care plans in conjunction with physician's orders and in coordination with interdisciplinary team members, based on patient needs.
    • Assess and re-evaluate patient needs, consulting with physicians, reporting changes, and communicating with others when a change in the Care Plan is necessitated to ensure optimal comfort and quality of life.
    • Provide education to patients, families, and caregivers about disease progression, symptom management, and end-of-life care options.
    • Collaborate with other healthcare professionals, including physicians, social workers, chaplains, and CNAs, to ensure a cohesive team approach to patient care.
    • Maintain accurate and up-to-date documentation of patient assessments, care plans, and interventions by practicing effective documentation at the bedside using Electronic Medical Records (EMRs) and prepares clinical progress notes on nursing services delivered in a timely manner.
    • Participate in interdisciplinary team meetings, providing valuable input and contributing to the overall care plan.
    • Stay current with best practices, research, and developments in hospice care, seeking continuous professional development opportunities.

    Requirements:

    • Current and unrestricted Registered Nurse (RN) license in the state of Virginia.
    • Minimum of 2 years of nursing experience preferred, preferably in hospice, palliative care, or a related field.
    • Knowledge of hospice regulations, practices, and standards of care with a demonstrated ability to manage all aspects of terminal care, health team service coordination, documentation, and case management.
    • Excellent clinical assessment and critical thinking skills, emotionally mature with the ability to cope with family stressors and tolerance of individual lifestyles.
    • Strong communication and interpersonal skills, with the ability to establish rapport with patients, families, and healthcare professionals.
    • Compassionate and empathetic nature, with a genuine desire to provide comfort and support during challenging times.
    • Ability to work independently and as part of a team, collaborating effectively with interdisciplinary professionals.
    • Highly organized, self-motivated with the ability to articulate information with a diverse population, and proficient in using electronic medical records (EMRs) and other healthcare software.
