- Minimum Education:
- BHP I (Paraprofessional): High School degree or equivalent and 4 years of experience providing mental health services
- BHP II: Certified Nursing Assistant (CNA), or certificate in mental health or AODA
- BHP III: Bachelor's degree in Human Services or related field
- Required Skills:

- Proficiency in documentation of services provided in an Electronic Medical Record

- Physical ability to participate in restraints of patients when necessary

- Valid Wisconsin driver's license and appropriate auto insurance coverage as required by North Central Health Care
- Use of a personal vehicle
- Maintains regular and predictable on-site attendance
- Works in other Acute Care Programs beyond designated home program as needed/directed, and performs the essential duties of the program being worked
- Provide or support treatment in groups or on an individual basis. Treatment includes self-care, basic living skills, leisure, social skills, coping skills, self-esteem, therapeutic arts/crafts, de-escalation, etc.
- Utilizes an Electronic Medical Record to document daily interactions, medication administration and services provided and completes all required clinical documentation adequately, and within required timeframes
- Assists in development of treatment planning and support to clinical team.
- Performs general patient care services including activities of daily living, serving meals, and medication administration/management
- Assists in patient admission, orientation, and discharge processes
- Promotes Person Centered Care in all interactions with patients/clients
- Collaborates with treatment team to prevent adverse outcomes and ensure treatment plan is followed
- Assists with department duties as assigned, including cleaning, sanitizing, maintaining supply inventories, checking refrigerator temperatures, etc.
- Demonstrate comfort and participation in, and utilizes provided training practices and mechanisms at all times in, providing client/patient de-escalation and physical restraint/seclusion measures as needed during behavioral health emergencies within the Acute Care department and other NCHC departments/programs
- Participates in emergency codes to ensure patient safety and staff security, utilizing trained Crisis Prevention Institute de-escalation and physical restraint techniques with teammates as necessary (Reflected in Very Heavy under Functional Demands)
- Utilizes on-call managers, appropriate chain of command, and NCHC policies and procedures as necessary to ensure quality of client/patient care and effective problem solving
