    Hospice Registered Nurse (RN)


    Job Summary:
    Constellation Health Services is seeking a compassionate and experienced Hospice Registered Nurse (RN) to join our team.

    As a Hospice RN, you will play a pivotal role in providing comprehensive and compassionate care to our patients, ensuring they receive the comfort, dignity, and support they deserve throughout their final journey.

    You will work closely with our interdisciplinary team to assess patient needs, develop personalized care plans, manage medications and treatments, and provide emotional support to patients and their families.


    Responsibilities:
    Deliver compassionate and comprehensive nursing care to patients facing end-of-life challenges.
    Conduct thorough assessments to identify patient needs and develop individualized care plans.
    Manage medications and treatments as prescribed by the physician.
    Provide emotional support and guidance to patients and their families.
    Educate patients and families about hospice care and end-of-life issues.
    Collaborate effectively with interdisciplinary team members to ensure coordinated care.
    Maintain accurate and up-to-date patient records.
    Participate in ongoing education and training opportunities to stay abreast of the latest advancements in hospice care.


    Qualifications:
    Current and active RN license in the state of employment.
    Bachelor's degree in nursing (BSN) preferred.
    Minimum of 1 year of experience in hospice care or end-of-life care setting.
    In-depth understanding of end-of-life care principles and practices.
    Exceptional communication and interpersonal skills.
    Ability to work independently and as part of a team.
    Compassionate and empathetic approach to patient care.
