    POSITION SUMMARY:

    This position is responsible for providing in-home mobile medication monitoring services to clients in need of skills training, symptom management, and resource coordination. The incumbent is responsible for providing mental and physical healthcare education and coordination of medication and related healthcare services; developing and implementing recovery-oriented medication management plans that are comprehensive, accurate and clearly tailored to each client's individual needs; and, conducting ongoing assessments to ensure that goal plans remain relevant, that progress, or the lack thereof, is closely monitored and that modifications can be made as deemed appropriate.


    MINIMUM QUALIFICATIONS:

    1. Rehab Tech 3 Bachelors degree in mental health related area and at least 12 months experience as Rehab Tech 1 or 2 within the agency; OR, associates degree with at least 3 years full time direct care experience within the agency; OR, high school diploma with at least 48 months experience as Rehab Tech 1 or 2 within the agency; OR, bachelors degree in mental health related area with 18 months of full time (or equivalent) from outside the agency; OR, associates degree in mental health related area with at least 30 months of full time experience from outside the agency.
    2. Rehab Tech 2 - Bachelors degree in mental health related area; OR, associates degree in a related field with 2 years full time (or equivalent) related experience; OR, high school diploma (or equivalent) with 18 months full time experience as Rehab Tech 1 within the agency; OR, high school diploma (or equivalent) and 3 years of related full time (or equivalent) experience from outside the agency.
    3. Rehab Tech 1 High school diploma or equivalent.
    4. Strong clinical skills with working knowledge of mental health system, psychiatric rehabilitation and recovery principles.
    5. Effective communication skills, both written and verbal, organizational skills, knowledge of practical self-care skills and ability to work with minimal supervision.
    6. Ability to effectively work with persons with mental illness and/or developmental disabilities and implement medication monitoring plans for their program participation.
    7. Act 33/34/31 and FBI Clearances.
    8. Valid driver's license with access to a vehicle during scheduled work hours.
