Job Summary
The Community Health Worker Specialist (CHW) will provide outreach coaching and health-related social support for patients of all ages providing basic education on wellness mental health brief coaching discrete skill building coordination in accessing community supports to address identified needs.Responsibilities
- Provide ongoing outreach coaching and health-related social support for patients of all ages.
- Provide basic education on primary care and mental health wellbeing discrete skill building coordination in accessing community supports to address identified needs.
- Foster collaborative relationships with providers other Summit Pacific Medical Center caregivers other community organizations which support our patients.
- Appropriately document submit charges for services provided.
